Healthcare in Spain: how the system works

Public system: Sistema Nacional de Salud (SNS).

Spain runs a tax-funded public health system, the Sistema Nacional de Salud, delivered by each autonomous region. It is generally very good, but access is tied to paying into Spanish social security or holding a specific entitlement — it is not automatic simply because you live there.

If you work in Spain and pay social security contributions, you and your dependants are covered by the SNS. UK State Pension recipients can register an S1 form, under which the UK funds their Spanish state healthcare. Some regions also offer a pay-in "convenio especial" scheme for residents who do not qualify another way. A UK GHIC only covers tourists and short visits — it is not a substitute for resident cover.

Is health insurance mandatory in Spain?

If you apply for a non-lucrative (non-working) visa — the usual route for retirees and remote Britons — Spain requires private health insurance from an insurer authorised in Spain, with full cover and no co-payments, for the visa to be granted. This is a hard requirement, not a recommendation.

Can British expats use Spanish public healthcare?

Yes, if you qualify — by working and paying social security, holding an S1 form as a UK State Pensioner, or joining a regional pay-in "convenio especial" scheme. It is not automatic just because you are resident, so many Britons hold private insurance as well or instead.
